Elgibility

This program is offered to all children who will be 4-years-old by September 1st.

Parents will need to gather the following documents:

• Immunization records - Form 3231

• Birth certificate

• Social security card*

• Vision, hearing, nutrition, and dental - Form 3300

• Health insurance card*

• Parents’ identification

  *requested but not required

Orientation will take place at the end of July. Parents must register online prior to attendance.
